With the home visits, I primarily treat rock climbing related musculoskeletal and neuromuscular injuries. However, I do also treat other sport related musculoskeletal and neuromuscular injuries on a case by case basis. Physical Therapy Initial Evaluation: Home Visit
Cost: $300* + .75 cents per mile that is traveled from my location to the patient's home and back. This is a 2-hour session. See below for details.

What to expect during the Home Visit Initial Evaluation Session:
This 2-hour Initial Evaluation session involves a thorough and comprehensive assessment of your medical history, mental health, stress levels, sleep history, your diet, and overall daily lifestyle habits. I will be reviewing your intake form thoroughly, learning about your injury, and getting to know you a bit more as a whole person.
This is followed by a detailed physical assessment to better understand your injury and limitations. I will take you through a static and dynamic posture assessment, followed by a series of range of motion and strength assessments, as well as functional movement tests to observe the quality of your movement, limitations of movement, as well as assess symptoms of pain. Additionally, I will utilize my hands to perform various manual therapy techniques as needed during the physical assessment. This may include, but is not limited to joint mobility assessment, soft tissue massage, trigger point therapy, passive range of motion, and myofascial release techniques. After I complete the evaluation, I will share any immediate findings and observations with you at the end of the session.
How to prepare for the Physical Therapy Initial Evaluation Session (Home Visit):
I will bring my portable treatment table, as well as my assessment and treatment supplies. I will need a space to set up the treatment table, (the table measures at 28” x 73”), as well as room to move around when performing the physical assessment. I can make small spaces work! We can also move outside if there is good weather! I am extremely flexible as everyone has a different space. 😊 Please wear comfortable clothes that allow for easy movement without restriction.
What will I receive AFTER the Home Visit Initial Evaluation session?
After the Initial Evaluation session is complete, I will take additional time to do the following:
I will thoroughly review the findings on a deeper level. You will receive a PDF document with information on the type of injury and/or ailments I believe you are experiencing based on the findings from the Initial Evaluation. This document will provide information on how the Physical Therapy treatment sessions will be utilized to guide you on your healing journey, and to help you flourish naturally. I typically take 3 business days to complete this document following the Initial Evaluation.
You will receive a detailed PDF document where I provide Wholistic Lifestyle Recommendations to optimize your health, and to help you flourish naturally. These recommendations will be based on your intake form, and the findings, as well as discussions we will have during the Initial Evaluation. I typically take 8 business days to complete this document following the Initial Evaluation.
What will I NOT receive AFTER the Home Visit Initial Evaluation?
Do not expect the Initial Evaluation session to be a “quick fix” for your injury. This session will not “heal” your injury. Your injury and or ailment(s), did not develop overnight. Therefore, healing does not happen overnight. To recover from your injury, you will have to put in actual effort and be ready to commit to your healing journey. This means working together through the Physical Therapy treatment sessions to specifically address your injury and/or ailment(s).
Do not expect us to get to the root cause of your injury and/or ailment(s) from just the Initial Evaluation session. Your injury and or ailment(s), did not develop overnight. Therefore, finding the root cause does not happen overnight. For us to find the root cause, you will have to put in actual effort and be ready to commit to your healing journey. This means working together through the Physical Therapy treatment sessions to specifically address your injury and/or ailment(s).
Do not expect a home exercise program after the Initial Evaluation session. The Initial Evaluation session is a discovery session, where I gather information about you as a whole person, and about your injury. This allows me to better advise you on what your healing protocol might look like. A home program is gradually developed and updated throughout your healing process through the Physical Therapy treatment sessions.
Do not expect the healing process to be easy. Healing is absolutely possible. BUT, healing takes time, effort, and mental fortitude from you and me, your Wholistic Physical Therapist. If you are not ready to put in the work to recover from your injury, please do not schedule a session.
Physical Therapy Treatment Session: Home Visit

These Physical Therapy treatment sessions are designed to guide you through your healing process. During these sessions, I will focus on educating and guiding you through Wholistic Physical Therapy treatments that are specific to your injury and needs. These sessions may include a combination of corrective posture training, therapeutic exercise, movement correction, proprioception training, mindset work, and additional recommendations to help you flourish naturally. Additionally, I will utilize my hands to perform various manual therapy techniques as needed to assist with the healing process. This may include, but is not limited to joint mobilization, soft tissue massage, trigger point therapy, passive range of motion, and myofascial release techniques. We will be working on getting to the root cause of your injury and/or ailment(s) through these sessions. Additionally, a home program that is specific to your needs will be developed and updated gradually as we work through the healing process.
How to prepare for the Physical Therapy Treatment Session (Home Visit):

That is a very good question!
I put in a lot of effort, time, and energy “behind the scenes.” Prior to your Initial Evaluation appointment, I will be reviewing every single answer on your intake form. During this process, I will be developing additional questions to ask during your appointment session regarding your injury, diet, and lifestyle habits that have been inspired by the information you provide or do not provide on the intake form. I will be reviewing the pertinent anatomy that is specific to your injury and situation, as well as developing the order of tests and measures that I want to take you through to better understand your injury. During the Initial Evaluation appointment, I will be working with you in real time to assess and better understand your injury and your situation from a wholistic perspective.
After the appointment, I will be taking time to document our session as required per the Physical Therapy Practice Act. Additionally, I will be working on developing your personalized report where I provide you with information on your injury, as well as a second personalized report where I provide Wholistic Lifestyle Recommendations to help you flourish naturally. I have provided detailed descriptions of these PDF documents in the Home Visit Initial Evaluation section above.
The work that I do before, during, and after your Initial Evaluation session takes about 10-11 hours in total. (This does not include the driving time to and from your location.) If the injury and or ailment(s) are complex, then it might require more of my time. The point is, I put in a lot of time and effort into each Initial Evaluation session that I perform. My goal is to provide every patient with exceptional care that gets to the root cause, not “rushed care” that puts a band-aid on symptoms.

(The previous session is either the Initial Evaluation or the previous Physical Therapy Treatment session.) This plan is implemented and sometimes adjusted in real time during the Physical Therapy Treatment appointment.
After the appointment, I will be taking time to document our session as required per the Physical Therapy Practice Act. Additionally, I will take time to develop or update your home program, depending on where you are in your healing process. The time I put in before, during, and after each Physical Therapy Treatment Session is about 3-4 hours. (This does not include the driving time to and from your location.)

I do not. I have chosen to be out of network with insurance companies as it allows me to provide extremely thorough care for my patients without being controlled by insurance regulations and reimbursement rates.
Why should an insurance company, who has not met you, and does not know your situation, decide what kind of treatment you need? Additionally, why should an insurance company be able to decide what kind of treatment I provide for MY patients? That makes zero sense to me.

For Telehealth appointments, I accept all major debit and credit cards. Additionally, I accept HSA/FSA cards and accounts.
For in person appointments, I accept all major debit/credit cards, HSA/FSA cards and accounts, as well as personal checks, and cash.
I can provide you with a receipt regardless of which payment method you choose.
If you are choosing a payment plan, it can be set up with either a debit/credit card or an HSA/FSA card.

Yes, absolutely! I understand that we are all in a different financial situation. I offer payment plans as low as needed on an individual basis. There is no interest on these payment plans. I want to be able to provide amazing care for my patients, while also making sure that my patients do not experience financial strain while healing. I mean it. Please discuss payment plans with me if needed.
